Registration Information

        
REGISTRATION REQUIREMENTS
Students with missing documents WILL NOT be able to continue with the registration process until the documents are provided

The parent or legal guardian MUST accompany all students wishing to register.
Students who reside with someone other than a parent or legal guardian must have a legal, notarized document signed by the parent or guardian giving the person the student is living with the ability to make decisions concerning the student on their behalf.

All students wishing to register must be able to show proof that they reside within the School Board designated school boundaries. This may be accomplished by providing a current utility bill, lease agreement or mortgage contract. NO CABLE OR CELL PHONE BILLS. This proof of address MUST be in the name of the parent/guardian. A Driver’s License is NOT acceptable as proof of address.

If transferring from a public school within Hillsborough County:
Signed Withdrawal Form (if transferring during the school year).
Proof of address – utility bill, lease agreement, or mortgage contract. NO CABLE OR CELL PHONE BILLS.
This proof MUST be in the name of the parent/guardian.

If transferring from a school outside of Hillsborough County, outside of Florida or a private school:
A copy of your withdrawal paper (if transferring during the school year) and a transcript or last report card. Seniors MUST provide an unofficial transcript upon enrollment if they plan to graduate with a Freedom High School Diploma.
Proof of address - utility bill, lease agreement, or mortgage contract. NO CABLE OR CELL PHONE BILLS.
This proof MUST be in the name of the parent/guardian.
Certified Birth Certificate (NOT a copy)
Social Security Card
Physical examination done by a Florida Doctor within 12 months prior to enrollment.
Immunization records on a Florida form.
Immunization records must include: 5 DPT, 4 Polio, 2 MMR, Hepatitis B series, 1 Td booster or Tdap booster. Varicella vaccine or date when student had the Chicken Pox.
